Pre-Employment Medical Form:

It is essential that all employees be in a fit state to carry out their duties at work. A responsibility is placed on Lahey Constructions Pty Ltd and employees to ensure the health and safety for all persons whilst at work.

As part of your employment with Lahey Constructions Pty Ltd, all new employees will be required to undergo a pre-employment medical examination when they start, at the expense of the company (please ensure all medical accounts are forwarded to Payroll mailto:payroll@lahey.com.au ).

The pre-employment medical form has a section that requires you to complete prior to attending your examination. Your Doctor is also required to complete a section & return it to our office marked ‘confidential’. If you are a site employee, please ensure that your Doctor has the facilities to conduct audiograms. Assistance with making a pre-medical appointment with a Doctor can be provided by Lahey, upon request.
